Prescriptions

The Sierra VillageHealth (HMO) prescription program is based on a formulary. This list contains FDA-approved brand name and generic covered drugs, which are provided at various copayment levels applicable to Sierra VillageHealth's specific prescription drug plan.

To view and print the formulary, click on the link to the left. This list, which is updated periodically, is also available to you by contacting Sierra VillageHealth.

Sierra VillageHealth has contracts with pharmacies that equal or exceed the CMS requirements for pharmacy access in your area, and has more than 45,000 contracted pharmacies in the network.

Sierra VillageHealth offers two mail order programs.
